Participation in CAD shows is optional for all dancers except Parker Dance Company members. If your dancer is NOT participating in the 2021 recital please complete and submit the form below. All forms must be received by April 13th to avoid any costume or performance fee charges. Once fees are charged, NO REFUNDS can be made.
